Cost of Services

No fees are charged. Donations are requested. Funding is provided by a grant from the Valley Area Agency on Aging. 

Care management is a Geriatric Assessment Program designed to assist “high risk” elderly to remain in their homes. The program accomplishes this through the process of assessing the needs of the client, securing services he or she needs, and following up to maintain or revise services as needed.

Care Management Goals

  • To improve a client’s level of functioning at home.
  • To meet each client’s needs through community based organizations.
  • To monitor the client’s progress and make referrals.

Eligibility Guidelines

To be eligible for care management services, persons must be 60 years or older and must reside in Shiawassee County.
In addition, the person must meet at least one of the following:

  • Experiencing complex issues and having difficulty obtaining services independently.
  • Unable to perform the necessary activities of daily living (ie. Shopping, personal care, housekeeping) and present support is not adequate.
  • Physician, family, and/or client is considering nursing home placement, but believes other alternatives may be feasible.
  • Currently residing in nursing home when there is the possibility of returning home or to family if services are arranged.

Care Management Process

Intake: A phone interview will be made to determine eligibility.
Assessment: The Care Management Team (a nurse and social worker) makes a home visit to evaluate the client’s needs.

Arranging Services: The Care Management Team, will assist the client in obtaining services and will monitor the delivery of services.

Reassessment: A phone interview will be made every 30 days and home visits every 90 days.

Referrals may be made by:
The person needing services, family, friends, physicians, service agency personnel, or nursing homes.
The referral source will be notified of the client’s eligibility.
